Support for residents in private hotels, rooming houses, caravan parks and supported residential services. Outreach Program provides short-term support, linkage and referral to mainstream and specialist services.

 

People aged 16 years and over who are frail, aged or disabled, homeless or at risk of homelessness, with complex care needs in the Cities of Maribyrnong, Hobson's Bay, Wyndham, and Brimbank.
